Client Financial Services
It is our privilege to serve you and your family.
To help you fully understand your account with us, we will provide information based on payment options. We hope this information proves helpful to you.
You will be asked to complete a financial agreement at the time you arrange for services at Easter Seals. Accurate billing information is essential to the proper handling of your account. Any change in billing information should immediately be brought to the attention of Client Financial Services Staff.
Easter Seals collects co-pays at the time of service. This policy applies to the following families:
- Insured by third party payer (HMO/PPO/KidCare/IDPA) that requires co-payment at the time of service.
- Receiving services at one of the Pediatric Specialty Centers.
- Receiving services outside of the Early Intervention system.
- May or may not have established a payment plan or financial assistance arrangement for services.
This does not apply to children enrolled in the Early Intervention program through Child and Family Connections. Co-Pays for that program are made directly to the State of Illinois Department of Human Services.
Estimated Cost of Treatment:
As part of the admissions process, we will provide you with an estimated cost of treatment, which will outline the approximate dollar amount you may be required to pay as your portion of the treatment fees.
Unless you have a medical card with Medicaid or participate in Early Intervention, a statement of your account will be sent to you on a monthly basis. This statement will reflect the outstanding charges to your account, any payment received, the source of payment, and the balance due from you and/or your insurance provider.
